An dorsal dermoid cyst in recurrent meningitis neonate ; a case report
Abstract
Introduction Recurrent bacterial meningitis is a rare phenomenon. Predisposing factors for recurrent bacterial meningitis can be broadly categorized into congenital and acquired conditions and further divided into anatomical abnormalities, immunodeficiencies, and chronic parameningeal infections. In this report, we present a case of dorsal dermal sinus detected in recurrent bacterial meningitis. Case A 3-month girl was admitted due to 2days of fever and poor feeding. She was full-term neonate, birth weight 3.18 kg. Blood test showed white blood cell count 13,240 μL; platelets count 385,000 μL; C-rective proterin 64.72 mg/L. CSF study showed WBC 11,200 /μL; Polymorphonuclear 85 %; RBC 1,600 /μL; Proterin 194.9 mg/dL; Glucose 53 mg/dL; Serum glucose 139 mg/dL. Mild and diffuse leptomeningeal enhancement was noted in Brain MRI. Oraganism was not cultured. She was treated Vancomycin, ampicillin, ceftriaxone for 18 days. On follow-up CSF study finding was improved. After 2 month later, She was visited to ER due to 3 days of fever, vomiting and irritability. Laboratory finding showed WBC count 16,600 μL; C-rective proterin 114.74 mg/L; CSF WBC 1,370 /μL; Polymorphonuclear 85 %; RBC 30 /μL; Proterin 40.4 mg/dL; Glucose 69 mg/dL. She was treated vancomycin, ceftriaxone and amikacin. Thoracic MRI was showed sacral dorsal dermal sinus and tethered spinal cord. Conclusion A 3-month girl was diagnosed recurrent meningitis and sacral dorsal dermal sinus and tethered spinal cord was detected. The authors report a case of dorsal dermal sinus detected in recurrent bacterial meningitis.
